Ensure ACPS Has Important Contact and Health Information: Verify Back-to-School Forms by Sept. 30, 2024

Ensure all your student’s needs are provided for by verifying their Back-to-School forms online! It is important for Alexandria City Public Schools (ACPS) to have updated contact and student health information so that your school can reach out to you and stay informed of any significant changes concerning your child’s health. This step is vital to safeguarding your child’s well-being, especially in the case of an emergency.

For families of students who attend a Community Eligibility Provision (CEP) school, completing the Income Survey ensures your school receives accurate funding for resources such as meals and enrichment programs. Families of students who do not attend a CEP school need to submit the Free and Reduced Priced Meals (FARM) form as it determines student eligibility for free or reduced-price school meals and other educational programs and services.

ACPS is ready to guide our families through the process of filling out back-to-school forms in order for the school division to have updated information on every student. The school division created helpful videos in English, Spanish, Amharic and Dari to ensure back-to-school forms are filled out accurately and on time. For families who do not have internet access or speak a language other than English or Spanish, ACPS staff are ready to help you complete printed forms. All forms are due by Mon., Sept. 30, 2024.
