Verifying Your Back-to-School Forms Is Easy! Make Sure to Meet the Sept. 30, 2024 Deadline

Verifying Back-to-School forms online is made easy as families can complete the required forms through PowerSchool.

• Information from last year remains in the system so, in some cases, all that is required is for families to review the forms for accuracy and make any needed updates.

• By updating your student’s health information, it safeguards your child’s well-being by keeping the school informed of any significant changes. 

The school division created helpful videos in English, Spanish, Amharic and Dari to ensure Back-to-School forms are filled out accurately and on-time.

• For families who do not have Internet access or speak a language other than English or Spanish, ACPS staff are ready to help you complete printed forms.

Funding and eligibility depend on all families completing Back-to-School forms.

• Completing the Income Survey — for families of students who attend a Community Eligibility Provision (CEP) school — ensures your school receives accurate funding for resources such as meals and enrichment programs. 

• Submitting the Free and Reduced Price Meals (FARM) form — for families of students who do not attend a CEP school — determines student eligibility for free and reduced price school meals and other educational programs and services.
