Verifying Back-to-School forms online is made easy as families can complete the required forms through PowerSchool.

Information from last year remains in the system so, in some cases, all that is required is for families to review the forms for accuracy and make any needed updates.

By updating your student’s health information, it safeguards your child’s well-being by keeping the school informed of any significant changes.
The school division created helpful videos in English, Spanish, Amharic and Dari to ensure Back-to-School forms are filled out accurately and on-time.

For families who do not have Internet access or speak a language other than English or Spanish, ACPS staff are ready to help you complete printed forms.
Funding and eligibility depend on all families completing Back-to-School forms.

Completing the Income Survey — for families of students who attend a Community Eligibility Provision (CEP) school — ensures your school receives accurate funding for resources such as meals and enrichment programs.

Submitting the Free and Reduced Price Meals (FARM) form — for families of students who do not attend a CEP school — determines student eligibility for free and reduced price school meals and other educational programs and services.
